Levi Lazar LNHA


Levi Lazar brings a wealth of experience to Cooperstown Center as an Administrator. Levi is a well-seasoned Administrator that has traveled across multiple states to stabilize struggling facilities. He created a better, more home-like atmosphere for the residents and staff members alike, and as a result, he achieved better outcomes for the residents. Levi has served as a mentor and educator to young new aspiring Administrators within Centers Health Care.  Levi is highly ethical, a role model, strives for excellence, and a dynamic leader. Levi fosters team-building and empowers others to their highest potential.

Lacey Rinker RN

Director of Nursing

Lacey is a graduate of SUNY Morrisville, where she earned an Associate Degree in
Nursing. She lives by the words of Mahatma Gandhi: Be the change you wish to see in the world. It’s healthcare that she finds the best opportunity to affect that change in the lives of Residents.

Jeorge U. Arculla DPT

Director of Physical Therapy

Jeorge graduated with a degree in Physical Therapy in 2004 from Far Eastern University- Dr. Nicanor Reyes Medical Foundation in Quezon City, Philippines. After passing the Philippine Board Exam for Physical Therapy, he worked in his hometown at an orthopedic specialty center in Southern Philippines where he also helped in rendering physical therapy treatment at no cost to a couple of local hospitals. Jeorge was also a volunteer in providing basic medical and physical therapy services in one of the indigenous and remote towns where access to medical needs are impossible for two years after passing the Philippine Licensure Exam.


In 2009, he decided to continue his career as a physical therapist in the United States. After passing the National Physical Therapy Examination, Jeorge began working as a Travelling PT and landed at Cooperstown Center where he has been since 2010.


Jeorge enjoys keeping himself active by jogging and biking. He also loves to hike and travel.

Hailee Pasa MSW

Social Services Director

Hailee holds a Bachelor of Arts in Early Child Education and looks forward to continuing her education. She has a special talent for creating and managing innovative and engaging activities for the residents.

Amy Rose

Recreation Director

“It has been my honor & privilege to have worked in the field of elder care for over 25 years.”


Amy’s primary goal is to offer creative & inspirational programing. Be it life enrichment activities done in groups or alone, social events or educational, visiting with a therapy dogs or shopping at our monthly facility farmers markets, she works to meet our residents where they are on life’s journey.


“Our seniors have taught me, it’s not what you’re doing as much as how you feel when doing these activities. This is why my focus has been to build & maintain a sense of community & encouraging the growth of a support system we all need to have a life filled with joy.”


In her leisure time, Amy works as a freelance artist & crafter. She enjoys gardening, renovating, and attending flea markets and antiques shows around Central New York. Amy is married with two children, four grandchildren, and an elderly therapy dog named “Speck”.

Megan Bosc BSW

Director of Admissions

Megan is often the first person you interact with when coming to Cooperstown Center and you’ll be happy she is. Megan brings with her 6 years of experience in Long Term Care/Skilled Nursing. She has served Cooperstown Center residents and their families for over 10 years and loves working there. When she isn’t working, you can find Megan hiking and snowboarding. But what she does most often is spend time with her family and her 3 beautiful children.

For the absolute highest quality of care, Cooperstown Center staffs medical providers on each unit who stay in constant communication with leadership throughout the day. Team collaboration is our forte, and this commitment helps us maintain an unbeatable continuum of care.

Our team also works hard to keep therapies as consistent as possible, tracking all communications through an advanced electronic medical record system. This ensures immediate transmission of information across the entire care network.

Coronavirus Update

We are aware of the Governor’s new guidance regarding nursing home visitors. We are as excited as you are to welcome you all back to our facility but need to ensure that we do so safely and in keeping with the guidelines outlined in the Governor’s letter. 

It is important to note that per the Governor’s guidelines we will not be permitted to open up to visitors until 5 days from the date of this notice at the earliest. In addition to this timing requirement, the Department of Health must also first approve our individualized reopening plan before allowing visitors.
